Elevate Evolve Behavioral Health is proud to accept Medicaid, Sentara and United Healthcare . You can always learn more about your coverage by contacting your provider directly, however, as a service to the families we serve, we offer free Verification of Benefits. 

 A verification of benefits is a statement from your provider that outlines the coverage provided by your plan. It includes your deductible, copays, and out-of-pocket maximum, as well as any restrictions on coverage for particular treatments. 

 You don’t need to have a VOB in order to proceed with your admission if you are paying out of pocket. However, a VOB is the only way to determine what your insurance provider will cover—and what your own financial responsibility may be—up front.
